Modified circumferential cerclage wiring for comminuted patella fracture fixation in Peru. Report of 4 cases

Abstract

Introduction: Comminuted fractures account for approximately 30-35% of patella fractures and 55% of these fractures require surgery. Although open reduction and internal fixation are considered the best treatment options, their indication depends on the patient's conditions.

Case presentation: This article reports the cases of four adults (three men and one woman) who were admitted to a tertiary care hospital in Lima (Peru) due to comminuted fractures of the patella. In all cases, the fracture was managed surgically using an innovative fixation technique (first described in 2019), which uses modified cerclage wiring and combines a multidirectional tension band system and a reinforcement using cerclage wires in a circular fashion. Outstanding functional outcomes were obtained in all patients (mean Oxford score of 41 points in the postoperative follow-up at 4 and/or 8 weeks).

Conclusion: Based on the satisfactory outcomes reported in the four patients and given the accessibility of the required osteosynthesis material, the modified circular cerclage wiring technique for fixation of comminuted patellar fractures could be a replicable option for the treatment of these fractures in Peru.
