Abstract

Acetabular fractures are complex injuries, usually associated with high energy trauma, that almost always have a poor outcome despite any selected treatment. We present a case report of a young patient who had an acetabular fracture as a result of severe multiple trauma caused by the explosion of a grenade. After the completion of the open fixation of the acetabulum, the patient presented post-traumatic osteoarthritis, surgically managed with a hip arthroplasty. The lack of adequate bone quality in the acetabulum, make it necessary to perform a neo-acetabulum along the initial arthroplasty surgery, by placing bone allograft in the true acetabulum position and having to consider a forecoming revision surgery. The patient had a hidden infection. A step by step with several surgery times was necessary, with placement of antibiotic spacers, removal of osteosynthesis material, placement of allografts in the acetabulum and raised the possibility of a subsequent final review after being sure that the infection was already controlled. We restored the true acetabulum and the center of rotation, improving the patient’s posture and gait
