Eficacia y seguridad de la enoxaparina en reemplazos articulares de cadera o rodilla. Una revisión sistemática de la literatura

Introducción: La tromboprofilaxis forma parte de los protocolos de tratamiento en reemplazos articulares y la enoxaparina es la molécula más empleada. Esta revisión de la bibliografía trata de establecer su eficacia y seguridad.
Materiales y métodos: Se llevó a cabo una revisión de la bibliografía de estudios clínicos en reemplazos de cadera o rodilla que incluyeran enoxaparina como un comparador. Se analizó la eficacia y la seguridad de todos estos pacientes.
Resultados: De un total de 199 referencias capturadas, se incluyeron 22 estudios clínicos aleatorizados con un total de 21.308 pacientes; 12 estudios eran de cadera (12.738 pacientes; media de edad: 58,1, y mujeres: 55,8%) y 10, de rodilla (8.300 pacientes; media de edad: 66,2 años, y mujeres: 64,8%). La mortalidad alcanzó el 0,30% en reemplazo de cadera (IC del 95%: 0,2-0,4%) y el 0,17% en rodilla (IC del 95%: 0,1-0,3%). Se presentó tromboembolia sintomática en el 0,5% (IC del 95%: 0,4-0,7%) y en el 1,0% (IC del 95%: 0,8-1,2) de reemplazos de cadera y de rodilla, respectivamente, mientras que el sangrado mayor ocurrió en el 0,75% (IC del 95%: 0,6-0,9%) y en el 0,86% (IC del 95%: 0,7-1,1%) de los pacientes.
Discusión: La enoxaparina es un medicamento seguro en la tromboprofilaxis de reemplazos articulares. Este trabajo puede servir de base para las instituciones que deseen montar programas de farmacovigilancia en cirugía ortopédica mayor.
