Measurement of coronal plane angle of anterior cruciate ligament during arthroscopy

Abstract

Introduction: The reconstruction of the anterior cruciate ligament (ACL) is a frequently performed surgery, with a wide interest in knowing how to evaluate the proper position and angulation of this ligament. An angle in the coronal plane of 63-76º has been reported in both healthy and post-surgical patients.
Objective: To determine the coronal plane angle of the ACL in vivo in Colombian adult population through arthroscopy.
Materials and methods: Study case series, where 47 adult patients were included who underwent knee arthroscopy between October 2018 and March 2019, obtaining intra-surgical images of the ACL with a lenses of 0º and 30º through transpatellar portal and anterolateral portal, ACL angle was measured in each arthroscopy. Regarding the descriptive analysis of the data, absolute frequencies and percentages were calculated for the qualitative variables, and means, modes, medians and standard deviations for the quantitative variables.
Results: The average angles of the ACL were 57.54º ± 6.78º and 71º ±
7.24º with the lenses of 30º and 0º respectively, in addition a constant angulation was evidenced regardless of gender and knee laterality, maintaining a normal distribution in the studied population.
Conclusions: The average of the angles with each lens is similar to that reported in the world literature, and that they did not vary much between lateralities and between men and women, and therefore it is recommended that the angulation in Colombian patients undergoing ACL reconstruction either 71.8º in the transpatellar portal with the 0º lens or 57.5º in the anterolateral portal with the 30º lens.
